Student Forms Open July 1

Note to parents

If parents want to fill out forms for their children, the parent should create/login to their own account then fill out the "Request to connect to your child's account" form, which can be found at the bottom of their portal home page.

Warning: No one should use the same device to create an account.

Our system has a weird glitch and the accounts will merge together. If someone cannot use a separate device, contact aly@kyymca.org for assistance.

How to Access Your Forms

To ensure that you are filling out the correct forms:

Students:

• Log into the portal:
• Go to the left navigation menu
• Select KYA
• Under that select overview (start here)
• Here you will find a checklist of your forms (green check marks will appear once the forms are filled out)
• Below the checklist are big blue boxes with links to each of the forms.

Parents:

• Log into the portal
• Go to the left navigation menu
• Select KYA
• Under that select overview
• Scroll down to the chart with your child’s name
• Click the links to fill out the forms
• As the forms are completed green check marks will appear

Overview of KYA Forms

All students, chaperones and volunteers will need to fill out the following forms before you can register your delegation:

• Info Form
• Health Form
• KUNA / KYA Code of Conduct
  • New for 2026, you can fill out the same form and it can be used for both conferences
• Need Based Scholarship Application | Optional 
  • Application deadlines vary by conference session. See deadlines in cart above.
• Student Y Membership | Optional
  • This is optional for students to purchase memberships and they can do this directly through the portal or advisors can purchase the memberships and be invoiced)
• Update School Form | Only fill this out if you are changing schools
  • The Update School form allows students who are switching schools to easily update their school.
  • You can also use this form if you are new to the system and want to add a school.
  • Warning:  If you are staying at the same school, do not fill out this form. Entering the same school as you currently attend can confuse the software and delete your school!
