Each year, as a member of your schools roster for KY YMCA programs, we require you to review and complete an Info Form.
You can access the form now. Please be sure to check all fields to ensure that the KY YMCA can provide the highest level of programming possible.
Tips:
- The same Info Form now works for students and adults, please complete it appropriately for one person.
- You may have to start by looking yourself up in your old school, that’s okay, you can change it later.
- Do not use Google Autofill, it doesn’t save properly (especially phone numbers) and often overwrites your information with the owner of computer’s information.
