County and School Information
This info form is NOT your program registration. This is just a once-per-school-year prerequisite for all programs for general information.

Our system works best when we can find your old contact record if you've participated with us prior to now. If we can look you up, your information will pre-fill, if we can't we'll create a contact record for you that you can use moving forward. IF YOU BELIEVE YOU SHOULD BE IN OUR SYSTEM, BUT YOUR NAME IS NOT POPULATING, PLEASE EMAIL ALY AT ALY@KYYMCA.ORG

This form works for our students and adults. 
If you are an advisor or chaperone who will be at KYA or KUNA, please complete the form with yourself in mind. If you are a parent, please complete the form with your child in mind.

By providing your phone number and email, you agree to receive text messages and emails from the Kentucky YMCA Youth Association. Message and data rates may apply. Message frequency varies depending on number of conferences and programs you attend. 

NOTE TO PAST PARTICIPANTS: 
  • Please use the county and school from the last time you participated. You will have the option to change to your current school later. 
NOTE TO NEW PARTICIPANTS: 
  • Select the county your current school resides in. 
  • Use an email that both parent and student can access*
  • Use the same email throughout your time at the Y. 
*We understand the concerns of raising a student in the 21st century and not wanting them to have open access to the internet. Here are some options to consider:
  • Create an email address together – in creating the email together it is something more likely for everyone involved to remember.
  • Keep the password – it may work for your family to allow your child to have an email, but not have access to the password.
  • Forward the emails from their account to yours – once their account is set up go into the settings and forward the emails to your desired address. This allows their freedom, but is still monitored.
  • Gmail users: set up a family account and manage it using their Family Link 
If you have changed schools this year (whether you have moved location or changed to a high school) you will need to use your previous year's information in the lookup below. The next page will allow you to change to the correct school.

For Example: I am currently a 9th grader attending Corbin HS, but last year, I was an 8th grader at Corbin MS

For this page, I will need to put Whitley in the county, and Corbin MS in the school.
On the next page, there is a toggle to update to the new school:
"These are correct for my current county and school"
Below that I would select: 

"No, I need to move my contact record to the correct school"

I will then be able to put in the correct county and school in new boxes that appear (for this example, Whitley and Corbin HS)




Out-of-State: Indiana is listed as a county, otherwise select Out-of-State.
Other: Select Other if your school does not appear in the next question as an option.

Legal first name. (IF YOU ARE ALREADY IN OUR SYSTEM, YOUR NAME MUST BE ENTERED THE EXACT SAME WAY.)  If you cannot find your legal first name and go by a different preferred name, try searching it. If your name is not here, please use the checkbox option below.

Legal last name. If you cannot find your legal first name and go by a different preferred name, try searching it. If your name is not here, please use the checkbox option below.

Be careful that when you type in your legal first and last name below, it does not add a space afterwards. If this happens, it will throw an error on the following page.
