Before submitting your online Student YMCA Registration, you must download and complete the member affiliation excel sheet here:

Once you complete your affiliation form, save it to your computer. You will be prompted to upload the completed affiliation sheet at the end of the Registration Form.

IMPORTANT!

Please be sure all addresses and e-mail addresses are correct and complete for mailing Live Y’er newsletters to homes.  If addresses are not included, members will not receive the Live Y’er.

 

Also, this year race and gender are included on the member affiliation sheet. This information is optional but appreciated as it helps inform us about our organization and assist us in obtaining grants.

 

It pays to send your registration form as soon as possible: $8.00 if received by 10/15/08; $10.00 after 10/16/08.
