Registration Guidelines

Read all guidelines before completing registration!

In order to complete online registration you must be able to download and upload excel (.xls) and word (.doc) files.

 Online registration must be completed all at one time. You cannot submit sections piece by piece. Entire registrations (including proposals and fees) must be turned in by the deadine for a delegation to be considered a World Class Delegation.

 

DEADLINE FOR REGISTRATION:

West Junior KUNA: February 4

East Junior KUNA: February 15 

**You must complete the downloaded forms in  sections 3, 4, and 5 before filling out the online registration**

1. OFFICIAL REGISTRATION FORM

Please complete school and advisor information thoroughly. We need this to contact you if we have questions about your registration, and to mail your conference materials to you.

Delegate Fee Summary

Lines 1-4: all delegates should be accounted for on these four lines, regardless of any financial assistance. For example, if you have 30 delegates attending and 4 are receiving financial assistance, fees for 30 delegates at the full cost should be computed on lines 1, 2, 3, and 4.

Line 5: deduct financial assistance awarded to student(s) in your delegation by the Kentucky YMCA.

Line 6: total delegate fees

 

Advisor Fee Summary

Line 1: show number of advisors requesting double lodging (rooming with another advisor from your school or another school assigned by the KY YMCA)

Line 2: show number of advisors requesting single accomodations

Line 3: deduct $95 for every 15 students in your delegation (e.g. 15-29 students- deduct $95; 30-44 students- deduct $190, etc.) if you submit your registration on time

Line 4: total advisor fees

 

2. DELEGATE SPECIFIED ROLES ROSTER

Use this form to identify specific roles which will be played by your students at KUNA (candidate, leadership team applicant, doorkeeper, Security Council representatives, press corps member).

 

3. DELEGATE ROSTER

Download this form here. Complete this form, save the changes as "yourschoolDelegateRoster" (example: FranklinCoDelegateRoster). Upload this form in the Delegate Roster section (3) in the form below. List all other delegates on this roster. Do not duplicate names listed on the specified roles roster!

4. OFFICIAL PROPOSAL FORM

Download this form here.  Complete this proposal, save the changes as "yourschoolProposal" (example: HendersonCoProposal), and upload this form in the Official Proposal Form section (4) in the form below.

 

5. HOTEL REGISTRATION FORM

Download this form here. Complete this form, save changes as "yourschoolHotelRegistration" (example: CaseyCoHotelRegistration), and upload this form in the Hotel Registration Form section (5) in the form below. This for is used to make your hotel rooming reservations. Please list all advisors and students. One male and one female room of three delegates are permitted at no extra charge; otherwise students are housed four to a room unless the additional fee is paid to have three in a room. Rooms of two delegates will be housed with another delegation. Also, please note special needs on this form (wheel chair accessibility, first floor, etc.)

 

6. PRINCIPAL APPROVAL FORM

Signing your name in the box provided in the form below lets us know that the students listed on the Conference Registration forms have approval from your Principal (or equivalent) to attend KYA. Also that your Principal (or equivalent) understands that adult supervision of the students at the conference is to be the responsibility of the teachers/sponsors from the school, in cooperation with the KY YMCA staff. Also, that your Principal (or equivalent) has reviewed the proposal(s) submitted for consideration and understands the terms of the Student and Adult Codes of Conduct signed by each participant. 

7. T-SHIRT FORM

Each delegation with on time registration will receive a free conference t-shirt. Please fill out your group's required sizes.

 

8. INTERNATIONAL STAGE RESERVATION FORM

If you wish to participate in the International Stage, please complete this form along with a description of your act. Remember, only the first 10  complete on time registrations with international stage reservations will be allowed to participate due to lack of time. Each school (not each country) will receive 5 minutes on stage.

 

9. LEADERSHIP TEAM AND CANDIDATE COMMITMENT FORM

Download this form here. Print this form out and send it in with your registration fee. It may not be submitted online because it must be signed. This form is to be completed by 11th grade candidates for Secretary General, 11th grade candidates for President of the Security Council, and 12th grade applicants for Leadership Team.

 

Forms 10-12 to be completed by all delegates and/or adults

Instead of making copies of these forms for both the KY YMCA staff and yourself, be sure to keep the originals for yourself at the conference, and sign this form and give it to us at check in. This is not considered a part of registration and does not need to be turned in earlier than when you check in at the hotel.

You will need to make copies of these forms for your student delegates and adult advisors. Bring these to KUNA. DO NOT MAIL OR FAX THESE FORMS!

10. Health/Medical/Publication Release Form

Download this form here. Each delegate should fully complete this form with his/her parents. Requires student and parent signature.

11. Student Code of Conduct and Dress Code

Download this form here. Note that this form is 2 pages. Please review this form with your delegates prior to the conference. Requires student and parent signiture.

12. Adult Code of Conduct

Download this form here.  Please as that all adult advisors carefully read this form before signing.

8) INTERNATIONAL STAGE RESERVATION (optional)

 

On the first night of the conference, we will host the "International Stage." This is a chance for delegations to perform cultural dances, rituals, songs, etc. before the entire assembly. All performances on the International Stage should showcase that country or countries' culture or heritage and thus should be professional and appropriate. It is NOT a talent or skit show. The YMCA staff has the righ to determine the appropriateness of a performance and whether or not it may be presented. Your complete registration with fees must be received on time in order to participate in the International Stage.

Because we will only have an hour to enjoy the International Stage, we must limit the number of delegations that participate. We must receive your complete registration on time in order to participate in the Internation Stage. Only 10 on-time delegations will be allowed to perform on stage. Each of these 10 delegations may have only one five (5) minute time slot on the International Stage. (So, if your delegation has two or more countries, each country can do a very brief presentation or one country can fill the entire five minutes.) If your delegation is note one of the 10 on-time delegations to register, you may still perform your cultural presentation at your booth in the Global Village. If more than 10 completed registrations are received by the deadline, 10 delegations will be chosen in a random drawing. If less than 10 registrations for rthe stage are received, we may notify your delegation that is can have addition time slots for other countries. Duplicate acts on the International Stage will be restricted. Each delegation will receive a notification via email as to wheter or not they will perform on the International Stage.

    

 

Please describe your delegation's International Stage act. This description should include cultural background on your performance, and how it represents the country/countries you are representing. International Stage acts that do not include descriptions will not be accepted.

 

 

All other forms can be downloaded in the above 'Registration Guidelines.'  Send in your Leadership Team and Candidate Commitment Form(s) (if applicable) with your registration fee as it requires signatures.

Remember, DO NOT MAIL OR FAX the Health/Medical/Publication Release Form, Student Code of Conduce and Dress Code, or Adult Code of Conduct. Bring these with you to the conference along with the Verification Certificate.

[Hopefully] Helpful Hints:

If when you click submit you do not see a 'Thank you' message at the bottom of the page, your registration may not have gone through. If this happens:

1) scroll up through your registration to make sure there is no red error text. This means you left something blank that is required. All fields with an asterisk (*) are required, even if the response is '0'.

2) check the box "Also send me a copy." As long as your e-mail address is correct in the 'advisor e-mail' field, you will be sent the same e-mail as I receive. If you don't receive an e-mail, neither will I.

3) Each time you resubmit, you will need to re-upload your delegate roster, proposal(s), and hotel form.

4) The security code below is case sensitive and is some combo of KY YMCA.
